Public Healthcare Companies in the UK: 1,600+ Active Firms (2026)
Public healthcare companies deliver regulated clinical, diagnostic and support services for public-sector health bodies across the UK.
Buying centres cluster around commissioners and operational leaders inside integrated care boards, hospital trusts, foundation trusts and local authorities. The most common commercial motions are contract-led rather than self-serve: pathway delivery, diagnostics capacity, clinical staffing, community care and outsourced operational support. Buyers tend to want providers with clinical governance, evidence of safe delivery, mobilisation capacity and the ability to manage multi-site workforces. Engagements are usually tendered or procured through public routes, with pricing tied to service volumes, staffing models and measurable service obligations.

The UK cohort comprises 1,603 trading companies, giving researchers a defined supplier universe rather than a broad healthcare count. Reported employment among headcount-reporting firms is 14,274 people, concentrated in operators with clinical teams, care staff, diagnostics personnel and contract management functions. The market sits close to public expenditure flows: ONS describes government-financed healthcare as the main financing route, including spending by the NHS, local authorities and other public bodies. That funding shape favours suppliers able to handle procurement scrutiny and service-continuity obligations.
England’s regulated activity regime is central to how these providers operate. Any organisation carrying out a regulated activity in England must be registered with the Care Quality Commission, and failure to register is an offence. Public-health contracting is shaped by the Provider Selection Regime, which relevant authorities use when arranging in-scope healthcare services. Integrated care boards, NHS trusts, foundation trusts and local authorities therefore assess not just price, but registration status, quality evidence, mobilisation risk and the transparency requirements attached to the selection route.
Commissioning cycles, workforce availability and governance evidence appear likely to shape the cohort more than product experimentation. Newer entrants tend to need time to build CQC track record, reference contracts and the operational routines expected by public buyers. Scale-ups with repeatable pathways or diagnostics capacity may find room where trusts and commissioners need external delivery capacity, but procurement scrutiny should keep expansion uneven. Consolidation may favour providers that can absorb mobilisation risk and maintain clinical standards across multiple sites.

Key facts
About 12% of the trading cohort reports turnover above £5M (205 of 1,603 firms) — the rest sits below that revenue band.
9% of the cohort was incorporated since 2022 (156 firms), so a sizeable share is in its first few filing cycles.
The Care Quality Commission requires any individual, partnership or organisation carrying out regulated activities in England to be registered under the Health and Social Care Act 2008.
The Provider Selection Regime has applied to in-scope healthcare-service contracting in England since 1 January 2024, with five selection processes and transparency requirements for relevant authorities.
ONS estimates put nominal UK healthcare expenditure at £317 billion in 2024, equal to 11.1% of GDP.
Government-financed healthcare accounted for £258 billion in 2024, or 81.3% of total healthcare expenditure.
ONS health accounts follow the System of Health Accounts 2011 framework, so the UK total is broader than health-service spending alone and includes some services treated domestically as social care.

How UK public healthcare companies work and how to sell to them
What they do
Revenue usually comes from commissioned care and capacity rather than product licences. Public healthcare suppliers are paid for staffed clinical services, diagnostics, community pathways, treatment support, care packages, managed facilities or back-office operations. Pricing is typically activity-linked: per test, appointment, shift, patient episode, care visit or service line, often with fixed mobilisation costs and service-level deductions. Larger contracts can resemble managed-service agreements, where the provider supplies clinical governance, workforce planning, reporting and operational management as one package. Smaller engagements tend to be spot purchases, framework call-offs or short-term insourcing work used to clear backlogs or cover workforce gaps.
Who they sell to
Buyers are mostly public-sector commissioners and provider-side operational teams: integrated care boards, hospital trusts, foundation trusts and local authorities. Decision groups tend to include commissioning leads, clinical directors, chief operating officers, finance directors, procurement teams and quality or safeguarding officers. Procurement is rarely a single-contact sale. Typical routes include public tenders, framework competitions, Provider Selection Regime processes and, for urgent capacity, more direct call-off mechanisms. Sales cycles often run for months because buyers test quality evidence, CQC status where relevant, mobilisation plans, information governance, workforce vetting and business continuity. Lower-risk purchases may start with a pilot, but larger awards usually require referenceable delivery and formal contract assurance.
What they buy
Most public healthcare providers tend to spend on systems and services that reduce operational risk rather than speculative growth tooling. Common needs include workforce management, rostering, payroll, HR, recruitment, clinical training, credential checking, safeguarding processes and agency spend control. Compliance and governance tools also matter: incident reporting, audit trails, policy management, CQC evidence packs, complaints handling and information-governance workflows. Operators with diagnostics or multi-site delivery often need scheduling, capacity planning, patient communications, data integration, analytics, cybersecurity, cloud infrastructure and secure device management. Non-software suppliers can find demand around clinical negligence cover, employment law, bid support, accounting, estates maintenance, transport, uniforms and specialist consumables.
Why and how to sell to them
Buying triggers tend to be visible before a formal tender lands: a new contract award, service mobilisation, site expansion, CQC inspection preparation, winter-pressure planning, a leadership change, headcount growth or a move into a regulated activity. Pain points are practical: filling clinical shifts, proving governance, keeping records audit-ready, protecting patient information and meeting service obligations without adding administrative load. Outbound works better when it maps to those risks in the buyer’s language. Claims about efficiency should be tied to mobilisation speed, staff utilisation, evidence capture, contract reporting or reduced procurement friction, rather than generic claims about modernisation.
How this list is built
Data sources
This list is built from UK Companies House filings, XBRL accounts data, and semantic analysis of each company's public website. Revenue and headcount figures come from the most recent filed accounts; where the company has not filed, values are estimated using a model trained on filed history and peer benchmarks and are labelled as estimates.
Classification
Rather than relying solely on SIC codes, Firmbase classifies each company semantically: the company's website is crawled, an AI model reads what the company actually sells, and the company is placed into the relevant industry and subsectors. SIC codes are used as one signal but not the only one. This means a company that registered under a generic SIC code but pivoted into (for example) fintech is correctly identified as fintech, not as its original SIC category.
Freshness
The underlying company data is refreshed from Companies House continuously; filings appear in the list within days of submission. The curated list ordering is regenerated when the underlying data moves meaningfully (company count changes by more than 5%, a new company enters the top-ranked segment, or the filed-revenue numbers for the top firms change). You can see the last-updated timestamp near the top of the page.
