Healthcare Companies in the UK: 123,754 Active Firms (2026)
Healthcare providers in the UK deliver regulated clinical, diagnostic and care services across public, private and mixed funding routes.
Buying centres sit mainly in clinical operations, commissioning, estates and care quality, rather than a single technology or procurement function. Providers sell capacity, staffed services and regulated pathways to public commissioners, private patients and mixed-funding buyers, with private contracts and publicly funded activity often sitting inside the same operating group. Typical commercial relationships are not small purchases: they involve care packages, diagnostic throughput, hospital or day-case capacity, or community services delivered through registered locations and accountable management structures. The cohort therefore mixes asset-heavy operators with specialist clinical providers whose growth depends on utilisation, referral access and compliance as much as sales.

123,754 healthcare companies are actively trading in the UK, giving the market a broad incorporated base that ranges from lean specialist operators to multi-site clinical and care businesses. Reported headcount across the cohort stands at 33,534 people, so the workforce footprint is not confined to owner-managed clinics. The shape of the list fits the wider UK mix of public, private and mixed-funding demand: a long tail of small providers sits alongside a narrower revenue-ranked group of larger hospital, diagnostic, day-case and adult social care operators.
English providers carrying on CQC-regulated activities must register with the Care Quality Commission before delivering those activities; doing so without registration is treated as an offence. Registration attaches to the legal entity, the regulated activity and the relevant locations, so corporate structuring matters as much as clinical scope. Applicants need to show fitness and compliance under the Health and Social Care Act framework and associated regulations, and some services need a registered manager as a condition of registration. Market structure is therefore shaped by permissioning, inspection and location-level accountability as well as referral flows and payer mix.
Growth paths appear easier for operators that can combine clinical governance with repeatable site-level delivery, because the registration model ties expansion to locations, managers and regulated activities. Smaller providers tend to face a heavier proportional burden from compliance, recruitment and inspection readiness, while larger groups can spread governance functions across multiple services. Consolidation has been a recurring route where buyers can add capacity without diluting local accountability. Demand is likely to remain mixed across public funding, self-pay and private routes, but margin pressure and workforce availability may keep many providers at modest scale.

Key facts
About 1% of the trading cohort reports turnover above £5M (1,239 of 123,754 firms) — the rest sits below that revenue band.
30% of the cohort was incorporated since 2022 (37,684 firms), so a sizeable share is in its first few filing cycles.
In England, health and adult social care providers carrying on a CQC-regulated activity must register with the Care Quality Commission.
The CQC states that providing a regulated activity without registration is an offence, with registration tied to the legal entity, regulated activities and locations.
ONS provisional estimates put total UK current healthcare expenditure at approximately £317 billion in 2024, including £258 billion financed by government.
Out-of-pocket expenditure accounted for 14.6% of overall healthcare spending in 2024, or £46 billion.
PHIN reported 238,000 private in-patient/day-case admissions in Q1 2024, 3% above the previous highest quarter and 4% above Q4 2023.

How UK Healthcare companies work and how to sell to them
What they do
Healthcare providers earn revenue by turning staffed capacity into chargeable care: consultations, diagnostics, procedures, residential or community care packages, and managed clinical pathways. Income typically comes through public commissioning, private payment and mixed-funding arrangements, often within the same operating group. Pricing tends to follow the unit of delivery, such as per appointment, per scan, per procedure, per bed night, per care hour or per contracted service line. Asset-heavy operators carry costs in premises, equipment, clinical staffing and compliance management; specialist providers may have fewer sites but still need governed pathways, referral access and accountable management. Commercial performance is therefore tied to occupancy, theatre or diagnostic utilisation, referral conversion and the ability to staff services safely.
Who they sell to
Most healthcare providers sell to a mixed buyer base rather than a single persona. Public commissioners and procurement teams usually care about capacity, pathway quality, reporting and continuity of service. Private patients and mixed-funding buyers tend to judge access, waiting times, clinical outcomes and price transparency. In larger accounts, decisions may involve a chief executive, finance director, medical director, operations director, procurement lead, estates manager and registered manager, with clinical governance teams influencing any service that affects care delivery. Sales cycles are usually longer where public funding, regulated activity or site-level service delivery is involved, and procurement may run through tenders, framework-style routes, direct referral relationships or negotiated contracts.
What they buy
Most healthcare firms tend to spend on systems and services that protect utilisation, staffing and compliance. Common software categories include patient administration, booking and referral management, care planning, clinical documentation, incident reporting, rota management, payroll, finance, billing, procurement, analytics and cyber security. Service suppliers often sell recruitment, training, CQC preparation, legal advice, accounting, insurance, marketing, facilities management, estates maintenance and clinical governance support. Infrastructure spend is shaped by sites and regulated activities, so premises, medical equipment, diagnostics capacity, telephony, cloud hosting and data protection can all matter. Smaller operators may prefer outsourced functions and lighter implementation; multi-site groups often need reporting consistency across locations and accountable managers.
Why and how to sell to them
Healthcare buyers tend to evaluate suppliers when capacity, compliance or workforce pressure becomes visible. Useful triggers include a new registered location, an added regulated activity, a change in registered manager, inspection preparation, a public contract opportunity, a referral backlog, private demand growth, leadership change or recruitment strain. Outbound messaging usually lands better when it links the offer to a named operational pressure: reducing missed appointments, filling rotas, evidencing CQC readiness, improving billing accuracy, shortening referral-to-treatment administration or giving managers clearer site-level reporting. Claims need proof, not theatre. Buyers in this sector are usually wary of tools that add work for clinicians or fragment the audit trail.
How this list is built
Data sources
This list is built from UK Companies House filings, XBRL accounts data, and semantic analysis of each company's public website. Revenue and headcount figures come from the most recent filed accounts; where the company has not filed, values are estimated using a model trained on filed history and peer benchmarks and are labelled as estimates.
Classification
Rather than relying solely on SIC codes, Firmbase classifies each company semantically: the company's website is crawled, an AI model reads what the company actually sells, and the company is placed into the relevant industry and subsectors. SIC codes are used as one signal but not the only one. This means a company that registered under a generic SIC code but pivoted into (for example) fintech is correctly identified as fintech, not as its original SIC category.
Freshness
The underlying company data is refreshed from Companies House continuously; filings appear in the list within days of submission. The curated list ordering is regenerated when the underlying data moves meaningfully (company count changes by more than 5%, a new company enters the top-ranked segment, or the filed-revenue numbers for the top firms change). You can see the last-updated timestamp near the top of the page.
