The commercial base still skews towards smaller providers, with a thinner band of firms operating at larger contract scale. That fits a market where employer access remains uneven and capacity pressure can sit alongside continuing demand for surveillance, absence support and fitness-for-work work.
UK occupational health sits within employer duties under health and safety law rather than a standalone licensing regime. HSE expects employers to prevent physical and mental ill health arising from business activities, with medical surveillance, fitness checks for certain roles and return-to-work risk reviews used where exposure, role requirements or a changed health condition make them necessary. Support for workers with health conditions or disabilities also overlaps with equality law. Market structure remains fragmented and capacity-constrained; gov.uk provider research describes small providers, vacancies and workloads close to or beyond normal capacity, while quality assurance rests partly on voluntary accreditation rather than a universal permit.
Employer demand appears to be pulled by sickness absence management, safety duties and the need to keep people in work where health conditions can be accommodated. Providers with clinical governance, accreditation readiness and employer-facing case management tend to look better placed than pure referral brokers, but clinician supply remains a constraint. Consolidation looks plausible where regional operators need broader coverage or digital triage, although the long tail of specialist providers may persist because many buyer needs are local, role-specific and relationship-led.

Rather than relying solely on SIC codes, Firmbase classifies each company semantically: the company's website is crawled, an AI model reads what the company actually sells, and the company is placed into the relevant industry and subsectors. SIC codes are used as one signal but not the only one. This means a company that registered under a generic SIC code but pivoted into (for example) fintech is correctly identified as fintech, not as its original SIC category.

The underlying company data is refreshed from Companies House continuously; filings appear in the list within days of submission. The curated list ordering is regenerated when the underlying data moves meaningfully (company count changes by more than 5%, a new company enters the top-ranked segment, or the filed-revenue numbers for the top firms change).
